Hello friends.
my fathers full name is haveri raghuvir raghav. (H. K. Raghav).
haveri is the name of his place, and raghuvir is middle name.
please clarify what to write as surname in passport in DS-156 Item 6. Should it be raghav ?

He has place name,middle name but does he have a first name or a last name? :rolleyes:
Also you mentioned you are writing his surname in his passport? Does he not have a name in his passport?:p

My father's name is H. R. Raghav.
My father's passport has his name with expanded initials, with the first name as Haveri, middle name is Raghuvir. So the last name is Raghav.
Note that there is no last name column in passport.
However, in DS-156 item 6 has a column : Surname as in passport.
What should he fill for Surname in DS-156 Item 6. Should it be Raghav ? Please clarify.

So as per his passport his last name is RAGHAV ( though its wrong ). To be consistent have Raghav as last name on the DS form also.

yes in my father's passport his name is haveri raghuvir raghav.
In DS-156, there is no such thing as LAST NAME.
The DS-156 form only asks for first name, middle name and SURNAME.
So in the DS-156 SURNAME column, i should fill RAGHAV correct ?
